Registered Nurse - Centralized Follow Up

About the role

Supports the healthcare team in providing centralized follow-up services to patients post-discharge. Works closely with primary care providers to ensure continuity of care.

Responsibilities

• Follow up with patients via phone or email to monitor their recovery and address any concerns.
• Document patient interactions and progress in the electronic health record system.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Provide education to patients on managing their health conditions and medications.

Requirements

• Bachelor's degree in Nursing from an accredited program.
• Current New York State Registered Nurse license.
• Experience in a clinical setting preferred.
• Strong communication and interpersonal skills.
• Proficiency in Microsoft Office Suite.
