Registered Nurse - Case Manager Transitional Planning - RN

About the role

As a Registered Nurse - Case Manager Transitional Planning, you will be responsible for coordinating care plans and managing transitions for patients in a transitional planning setting.

Responsibilities

• Develop and implement care plans for patients transitioning between levels of care.
• Collaborate with interdisciplinary teams to ensure continuity of care.
• Assess patient needs and develop individualized care plans.
• Coordinate referrals and communication among healthcare providers.
• Monitor patient progress and adjust care plans as needed.

Requirements

• Bachelor’s degree in Nursing (BSN).
• Current licensure as a Registered Nurse (RN) in Minnesota.
• Experience in case management or transitional care preferred.
• Strong communication and interpersonal skills.
• Ability to manage multiple tasks and prioritize effectively.

Pay

The salary range for this position is $50,000 - $60,000 annually, commensurate with experience.
