Registered Nurse - Case Manager - Transitional Planning

Responsibilities

• Facilitate communication between providers, patients, families, and external agencies
• Lead education on case and utilization management
• Promote quality clinical, financial, and patient satisfaction outcomes
• Support best practices through committee work, research, and community involvement

Qualifications & Skills

• Bachelor’s degree in nursing from an accredited program
• At least one year of RN experience
• Certified Registered Nurse (RN) license
• BLS certification
• PREFERRED: Certified Case Manager (CCM) or Advanced Case Management - RN (ACM-RN)
