Registered Nurse Case Manager – Telephonic & Virtual Care

Role: Registered Nurse Case Manager responsible for coordinating comprehensive patient care. Key Responsibilities Conduct telephonic and virtual assessments to evaluate patient needs.Develop and implement individualized care plans to improve health outcomes.Collaborate with physicians, pharmacists, and community providers to ensure coordinated care.Educate patients and caregivers on health management and available resources.Connect patients with community and pharmacy resources for additional support.Advocate for cost-effective, appropriate care tailored to patient needs.Utilize clinical guidelines, data, and electronic tools to monitor progress and close care gaps.Support safe care transitions while ensuring regulatory compliance. Qualifications & Skills Current RN license with relevant clinical experience.Strong communication, assessment, and care planning skills.Ability to collaborate effectively with multidisciplinary teams.Proficiency in using electronic case management tools and data analysis.Knowledge of healthcare regulations and compliance standards.
