Registered Nurse Case Manager

Key Responsibilities

• Manage and coordinate care for home health and hospice patients.
• Perform comprehensive patient assessments and develop individualized care plans.
• Collaborate with physicians, interdisciplinary team members, patients, and families.
• Provide skilled nursing care, patient education, and family support.
• Maintain accurate and timely clinical documentation in the EMR system while ensuring compliance with regulatory standards.
• Participate in an on-call rotation, including occasional evenings and weekends.
• Deliver compassionate, patient-centered care while promoting positive clinical outcomes.

Qualifications

• Current and active Idaho Registered Nurse (RN) license.
• 1-2 years of nursing experience preferred; home health or hospice experience is highly preferred.
• Strong clinical assessment, organizational, and communication skills.
• Able to work independently and effectively manage a patient caseload.
• Compassionate approach to patient care.
• Valid driver's license and reliable transportation.

Schedule

Full-Time
Monday-Friday, 9:00 AM - 5:00 PM
Occasional weeknight and weekend on-call responsibilities
