About the role
Alternate Solutions Home Health is seeking a Registered Nurse Case Manager (RNCM) to lead patient care, coordinate multi-disciplinary teams, and ensure high-quality, cost-effective, and compliant services. The role involves direct clinical care, care planning, patient/family education, and supervision of care delivered by nurse aides and LPNs.
Responsibilities
- Oversee the overall care of the patient from beginning to end of episode
- Update all doctor orders in the patient chart accordingly
- Notify the physician of any significant changes in the patient’s condition
- Manage multi-disciplinary care as applicable
- Provide skilled nursing care in a patient’s home as ordered by the attending physician
- Observe and monitor patient conditions
- Perform OASIS assessments to develop and adjust individualized plans of care
- Administer medication as prescribed by the physician
- Decrease re-hospitalizations by front-loading visits for high-risk patients
- Promote continuity of care with appropriate admissions, transfers, and discharges
- Counsel patients and families on disease/injury processes and management
- Incorporate patient and family in the development of the plan of care
- Teach patients preventative measures and decision-making to reduce re-hospitalizations
- Listen to patient and family members
- Oversee and supervise total care provided by nurse aides and LPNs
- Evaluate treatments and medical condition according to the plan of care
- Determine if the level of care meets the patient’s needs
- Communicate supervisory visit results to LPNs and nurse aides
- Complete all clinical documentation per agency protocol and Medicare/Federal guidelines
- Maintain an active RN license
- Communicate schedule changes to the scheduler
- Participate in all on-call requirements and case conferences
- Attend in-service trainings and mandatory agency meetings
- Drive to patient locations per scheduled visits
- Ensure daily attendance at assigned visit locations
- Document all aspects of subsequent, discharge, eval/recert/resumption of care visits within 24 hours
- Document all aspects of start of care visits within 24 hours
- Complete and submit all required documentation within specified company requirements
- Follow the plan of care within the RN scope of practice
- Fulfill RN visits in assigned geographic location per patient need within federal and state specifications
Qualifications
- Minimum of one year of experience as an RN in an acute care setting
- Home care experience is desired
- Ability to effectively communicate and create positive impressions with patients, families, physicians, and co-workers
- Ability to remain calm, patient, and accommodating
- Compassionate and caring demeanor when working with patients
- Knowledgeable of nursing best practices
- Ability to make appropriate nursing judgments
- Ability to identify situations and handle them with the best possible solution
- Detail-oriented and observant
- Disciplined work ethic with the ability to prioritize and be timely
- Ability to follow directions and work as a team member
- Valid driver’s license and auto insurance with your name as a listed driver
Education and Credentials
- Registered Nurse with current license in the state of employment
