Registered Nurse Case Manager, Level 2

About the role

A registered nurse case manager level 2 is responsible for managing patient care plans, coordinating services, and ensuring compliance with healthcare regulations. This position requires excellent communication skills, strong organizational abilities, and the ability to work independently and as part of a team.

Responsibilities

• Develop and implement patient care plans based on medical needs and preferences
• Coordinate services between various healthcare providers and agencies
• Assess patient needs and make recommendations for further treatment or referrals
• Monitor patient progress and adjust care plans as necessary
• Ensure all services provided are in accordance with healthcare regulations and standards
• Communicate effectively with patients, families, and other healthcare professionals

Requirements

• Bachelor's degree in nursing from an accredited program
• Current California RN license
• At least 1 year of relevant clinical experience
• Experience in case management or related field
