Registered Nurse Case Manager - Hospice

All Saints Hospice is seeking a passionate, dedicated Hospice RN to join our team in Alexandria, LA. Our hospice RNs provide expert, patient-centered care at a critical time in people’s lives.

- As a RN Case Manager, you will:
- Assess/monitor physical, emotional, and psychological needs of patients
- Create hospice care plans that align with the patient's wishes and goals
- Direct nursing care: administering medications, treatments, and interventions
- Pain and symptom management
- Education and support for the patient’s family and caregivers
- Collaborate with interdisciplinary team
- Maintain accurate and timely documentation
- Participate in on-call rotation as required by local branch
Requirements
- Graduate of an accredited nursing school with current licensure to practice in state of operation
- One year nursing experience required, 3+ years preferred
- Experience in hospice or a similar setting preferred
- Knowledge of the hospice philosophy of care
- Commitment to clinical and documentation excellence
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
Qualifications
- Must have a valid driver’s license
- Must have a vehicle that is insured in accordance with state/or organization requirements and is in good working order

- Ability to work independently and as part of a team
- Excellent organizational and documentation skills
