Registered Nurse Case Manager - Hospice

About the role

Adoration Hospice, an affiliate of BrightSpring Health Services, is seeking a passionate, dedicated Hospice RN to join the team in Chattanooga, TN. Our hospice RNs provide expert, patient‑centered care at a critical time in people’s lives, offering 1:1 care to make a lasting impact on patients and families while working in a supportive, fulfilling environment.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient’s wishes and goals.
• Provide direct nursing care, including administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on‑call rotation as required by the local branch.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in the state of operation.
• At least one year of nursing experience (three or more years preferred).
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an insured automobile that meets state/organization requirements and is in good working order.

Schedule

Monday through Friday, 8 am – 5 pm.
