Role Overview: Provides comprehensive home health nursing care, including case management, post-hospital assessments, discharges, patient education, dressing changes, IV therapy, and access site care, ensuring safety and appropriate support. Responsibilities: Performs skilled nursing duties per policy, assists with home safety needs, manages documentation via Epic system, and offers patient teaching on public programs and safety measures. Qualifications & Skills: Graduation from an accredited nursing program, current state licensure, valid driver’s license, and CPR certification. Preferred: at least one year of related experience. Position Type: Per diem, providing visits on an as-needed basis, aligned with departmental policies.
