Registered Nurse - Case Manager, Home Health

Role Overview: Provides comprehensive home health nursing care, including case management, post-hospital assessments, discharges, patient teaching, dressing changes, IV therapy, access site care, and general skilled nursing according to policies. Key Responsibilities Ensures patient safety and appropriate care by assisting with needs like lifeline, meals on wheels, and public program instruction. Maintains accurate documentation using the Epic system. Performs visits on an as-needed basis, following department policies, while upholding the organization’s mission, values, and standards. Qualifications & Skills Requires graduation from an accredited nursing school, current state licensure, a valid driver’s license, and CPR certification. Preferred candidates have at least one year of related experience.
