Registered Nurse Case Manager - Full Time

About the Role

This position involves providing personalized, in-home nursing care to patients, focusing on recovery, safety, and independence. The nurse conducts comprehensive assessments, delivers skilled interventions such as wound care, infusions, and medication management, and educates patients and caregivers to promote self-care.

Responsibilities

• Serve as primary clinical partner for patients and families
• Perform in-home assessments and develop care plans
• Implement skilled nursing interventions tailored to individual needs
• Educate and empower patients and caregivers
• Monitor patient progress and communicate with care teams
• Coordinate care and ensure continuity
• Complete accurate clinical documentation to ensure quality and compliance

Requirements

• Active RN license in the practicing state
• At least one year of clinical nursing experience
• Patient-focused, organized, and motivated to deliver high-quality care
• Valid driver’s license and reliable transportation
• CPR certification (or ability to obtain)
