Registered Nurse Case Manager Full Time

About the role

We are seeking a dedicated Registered Nurse (Home Health) to provide personalized, one-on-one nursing care in patients' homes.

Responsibilities

• Conducting comprehensive assessments
• Delivering skilled interventions such as wound care, infusions, medication management, and post-operative support
• Developing patient-centered care plans
• Educating and empowering patients and caregivers
• Monitoring progress, identifying early changes, and coordinating with interdisciplinary teams to ensure quality outcomes and prevent hospitalizations
• Accurate documentation and effective communication

Qualifications

• Active RN license in the practice state
• At least one year of clinical nursing experience
• Patient-focused, organized, and motivated to deliver high-quality care
• Valid driver's license, reliable transportation, and auto insurance
• CPR certification or ability to obtain
