Registered Nurse Case Manager - Field in Northern Virginia

About the role

We’re looking for a compassionate and motivated Registered Nurse Case Manager to join our team, supporting Northern Virginia. This is a field-based role that blends in-person visits with telephonic outreach to deliver coordinated, patient-centered care.

Location Specifics: Fairfax County, and surrounding areas with preference for those in Fairfax, Alexandria, Arlington, Annandale, McLean, Vienna, Falls Church, Herndon, Reston, Sterling and Springfield.

Responsibilities

• Conduct comprehensive assessments (telephonic and face-to-face) to evaluate members’ medical, behavioral, and social needs
• Develop and implement personalized care plans that promote both short- and long-term wellness
• Coordinate care across providers, programs, and services to ensure a seamless experience for members
• Use clinical expertise and data insights to identify risks, gaps in care, and opportunities for intervention
• Support members with complex or multiple conditions by applying a holistic, whole-person approach
• Facilitate referrals to appropriate clinical and community resources
• Participate in case conferences and collaborate with multidisciplinary teams to optimize care strategies
• Review claims history and benefit eligibility to inform care planning
• Engage members using strong interviewing skills to build trust and encourage active participation in their care
• Ensure all case management activities comply with regulatory and organizational guidelines

Requirements

• Active, unrestricted Registered Nurse (RN) license in Virginia
• Residence in Northern Virginia in one of the specified locations
• Reliable transportation and willingness to travel up to 75% of the time (mileage reimbursed)
• 3+ years of clinical nursing experience (e.g., medical-surgical, acute care, or behavioral health)
• Strong communication, organization, and critical-thinking skills
• Comfort working with technology, including multiple systems and documentation platforms

Preferred Qualifications

• Experience in case management, care coordination and/or managed care organizations
• Discharge planning and/or transition of care experience
• Bilingual (English/Spanish)
• Ability to thrive in a fast-paced, dynamic environment and manage multiple priorities effectively

Education

• Associate Degree in Nursing (required)
• Bachelor of Science in Nursing (BSN) (preferred)

Schedule

• Monday–Friday, 8:00 AM – 5:00 PM EST
• 40 hours per week
• Full time

Pay

The typical pay range for this role is $60,522.00 - $129,615.00 annually. This range represents the base salary and may vary based on experience, education, geography, and other relevant factors. This position is also eligible for a CVS Health bonus, commission, or short-term incentive program in addition to the base pay.
