About the role
The ICM Case Manager develops proactive care plans to improve member outcomes and overall wellness, primarily serving dual eligible populations with complex health and social needs.
This is a remote role requiring a dedicated workspace, with responsibilities including assessing member needs, coordinating care, and collaborating with teams to promote wellness.
Responsibilities
- Evaluate member needs using clinical tools and data review.
- Apply clinical judgment to reduce risks and address barriers impacting care.
- Conduct holistic assessments considering multiple diagnoses and social factors.
- Collaborate with healthcare teams and stakeholders to overcome barriers and meet goals.
- Utilize case management processes compliant with regulations and policies.
- Engage members through motivational interviewing to discern health needs.
Requirements
- Minimum of 3+ years of clinical practice experience.
- Active RN licensure in NJ.
- Reliable transportation and willingness to travel 25-50%.
Skills
- Strong analytical, organizational, and communication skills.
- Proficiency with computer software including MS Office and proprietary systems.
- Preferred: Certified Case Manager, bilingual skills, and experience in care management or discharge planning.
