Responsibilities
- Care coordination for hospitalized patients
- High-risk screenings and assessments
- Collaborating with multidisciplinary team members
- Coordinating post-acute care services with proper documentation
Requirements
- 1 year case management experience
- Associate's Degree Nursing
- Basic Life Support (BLS)
- Driver's License (DMV)
- Active licensed Registered Nurse (RN)
