Registered Nurse - Case Manager

Spectrum Healthcare Resources has a potential opportunity for an experienced Registered Nurse – Case Manager to support the Trainee Health Squadron at Reid Medical Clinic. Join a dedicated healthcare team supporting the health and readiness of our military community.

Schedule

• Monday–Friday
• No nights, weekends, or federal holidays

Requirements

• U.S. Citizenship
• Associate Degree in Nursing or higher from an accredited program
• Current, unrestricted RN license meeting Texas requirements
• 3 years of RN Case Management experience within the last 4 years
• Current BLS certification (American Heart Association or American Red Cross; hands-on training)
• Case Management certification such as CCM, CDMS, CRRN, COHN, RN-NCM, or another PWS-approved certification

Responsibilities

• Coordinate individualized patient care and treatment plans
• Manage referrals, follow-up care, and continuity of services
• Collaborate with healthcare providers and community resources
• Educate patients and families on treatment and wellness
