Registered Nurse - Case Manager

About the role

A Case Management Nurse assesses, develops, implements, and evaluates discharge planning needs for patients in collaboration with an interdisciplinary team.

Responsibilities

• Ensures compliance with discharge planning standards
• Identifies barriers to length of stay
• Assesses high-risk patients for readmission
• Collaborates on discharge plans with the interdisciplinary team
• May handle utilization management, ED case management, and admission processes
• Liaises with post-acute providers
• Applies evidence-based practices and adheres to professional nursing standards
• Maintains patient safety

Requirements

• Graduate of an accredited nursing program
• 3+ years of acute care experience
• Active RN licensure in relevant states
• Excellent communication and attention to detail
• Self-motivated
• Physical ability to perform typical nursing duties

Skills

• Strong communication and critical thinking
• Collaboration with interdisciplinary teams
• Knowledge of healthcare protocols and patient care processes
