About the Role

The Home Care Registered Nurse Coordinator plans, organizes, and implements in-home services for elderly participants, acting as a liaison between home health agencies and the PACE program. Responsibilities include assessing participants, coordinating care plans, managing interdisciplinary team collaboration, and utilizing telehealth and community resources to promote health and wellness in home environments.

Qualifications

• Bachelor of Science in Nursing or agreement to obtain it within 6 years.
• Current RN licensure.
• Valid driver’s license and reliable transportation.
• At least 2 years of experience in interdisciplinary teams.
• At least 1 year of experience working with frail or elderly populations.

Preferred experience includes community-based nursing, familiarity with EMR, and mental health or ACTT RN experience.
