Registered Nurse - Case Manager

About the role

Provide in-home care to former Nuclear Weapons Workers with chronic or terminal illnesses caused by exposure to toxic substances.

Responsibilities

  • Conduct weekly in-home nursing assessments, including vitals and physical exams
  • Ensure home safety, manage medications, and oversee caregiver services
  • Develop and evaluate patient care plans, collaborate with team members, and serve as a clinical resource
  • Assist with staffing, training, documentation review, and quality improvement activities
  • Participate in client care conferences and support client health and well-being

Requirements

  • Graduate of an approved nursing program (BSN preferred)
  • Current, unrestricted RN license
  • At least two years of nursing experience, including one year in home care
  • CPR certification; supervisory or case management experience is a plus

Skills

  • Strong assessment and communication skills
  • Ability to collaborate with healthcare teams and manage patient care

Pay

Competitive hourly rate of $47 plus shift differentials.
