Registered Nurse - Case Manager

About the Role

This position involves assessing, developing, implementing, and evaluating discharge planning needs for patients in collaboration with an interdisciplinary team. The nurse ensures compliance with discharge regulations, identifies and addresses barriers to patient care, and manages high-risk readmissions by developing appropriate care plans. Responsibilities may include utilization and case management processes, acting as a liaison with post-acute providers, and contributing to quality improvement efforts. The role requires applying evidence-based practices, adhering to professional nursing standards, and demonstrating patient-centered care based on age and clinical needs.

Responsibilities

• Assess and coordinate patient discharge plans to ensure seamless transition to the next care level
• Identify gaps and barriers affecting length of stay and readmission risks
• Collaborate with multidisciplinary teams and post-acute providers
• Ensure compliance with discharge planning standards and utilize evidence-based practices
• Participate in quality improvement initiatives and perform other assigned duties

Requirements

• Graduate of an accredited nursing program or equivalent
• 3–5 years of acute care, medical/surgical, or ICU nursing experience
• Registered Nurse (RN) license in Missouri or a Compact State
