Registered Nurse - Case Manager

Role: RN Case Manager responsible for patient-centered care coordination to improve health outcomes. Responsibilities include: Assessing, planning, implementing, and evaluating care options; developing personalized treatment plans; collaborating with patients, families, and healthcare teams; monitoring progress through clinical and utilization data; promoting early intervention; and ensuring documentation compliance. Qualifications and Skills: Minimum of 2 years RN experience; preferred certifications in case management (ACMA); BSN preferred; knowledge of healthcare laws, regulations, insurance policies, and healthcare IT; ability to work independently and in multidisciplinary teams. Work Environment: Full-time, primarily remote in Pennsylvania, with an exempt/salaried status.
