Registered Nurse Case Management Coordinator
About the role
The RN Case Manager plays a crucial role in enhancing customer service, ensuring high-quality care, and optimizing hospital reimbursements. This multifaceted position involves reviewing patient benefits for planned admissions, conducting concurrent chart reviews for medical acuity, coordinating patient admissions to supportive care units, participating in discharge planning, assisting in the assignment of DRGs, and interfacing with various hospital departments and external agencies.
Responsibilities
- Reviewing patient benefits for planned admissions
- Conducting concurrent chart reviews for medical acuity
- Cooking patient admissions to supportive care units
- Participating in discharge planning within the multidisciplinary care team
- Aiding in the assignment of DRGs
- Contacting hospital departments and outside agencies
Requirements
- Required Education: Associate’s Degree in Nursing or equivalent experience
- Prioritize five years of acute care nursing experience
- Preferred Education: Bachelor's Degree in Nursing
- Advanced computer coursework recommended
- Professional Experience: Prior related work experience in Patient Review or Case Management preferred
- Licenses: Current license as a Registered Nurse in good standing from the State of Wisconsin
- Computer Competency: Basic Microsoft Office Suite skills, including the ability to open documents, change fonts, save documents, and write letters

Competitive pay rate.

Schedule Details
- Week 1: 24 hours
- Week 2: 16 hours
- Variable between Monday - Friday
- Shift: 8:00 a.m. – 4:30 p.m.
- No weekends
