About the role

As a key member of the Viral-Specific T-Cell (VST) Program, the Care Manager (Navigator) will serve as a central point of contact for patients and families, providing comprehensive care coordination and education throughout the VST treatment process.

Responsibilities

  • Coordinates care across the continuum for patients receiving VST therapy, including pre-treatment workup, infusion, and post-infusion monitoring.
  • Serves as the primary liaison between patients/families and the multidisciplinary care team (physicians, advanced practice providers, nurses, pharmacists, and research staff).
  • Provides patient and family education regarding VST therapy, treatment expectations, side effects, and follow-up care.
  • Assesses patient and family needs related to psychosocial, financial, and logistical barriers to care and connects them with appropriate resources.
  • Facilitates communication with referring institutions and community providers to ensure seamless transitions of care.
  • Maintains detailed patient records and documentation in the electronic medical record and program databases.
  • Participates in program quality improvement initiatives and data collection efforts.

Qualifications

  • Bachelor of Science in Nursing (BSN) from an accredited nursing program.
  • Current unrestricted Registered Nurse license in the State of Ohio.
  • Minimum of 3 years of nursing experience, with at least 1 year in hematology/oncology or bone marrow transplant.
  • Experience in care coordination, case management, or patient navigation.

Skills

  • Strong clinical assessment and critical thinking skills.
  • Excellent interpersonal and communication skills with ability to educate complex medical information to diverse audiences.
  • Ability to work collaboratively in a multidisciplinary team environment.
  • Proficiency with electronic medical records and standard office software.
  • Strong organizational skills and ability to manage multiple priorities.
