Registered Nurse - Care Manager

Summary

The RN Care Manager will provide direct patient support both in-person and remotely. They will orchestrate care management across the continuum, coordinate care needs with physicians and care providers, develop care plans, implement interventions, and educate patients on chronic condition management. The RN Care Manager will act as part of an interdisciplinary care team.

Responsibilities

• Provide patient directed and disease specific self-management education and psychosocial support
• Monitor and measure patient progress and ensure timely and complete documentation following any patient-related contact
• Work in settings across the care continuum including hospitals, provider practices, and patient's home or community settings
• Collaborate with all members of the care team to assess, facilitate, plan, and advocate appropriate services to meet the patient’s health requirements on an individual basis
• Facilitate quality, cost-effective medical management

Required Qualifications

• BSN (preferred) or Associate’s Degree in Nursing (ASN) with a Bachelor’s degree in another discipline
• Five (5) years clinical experience in care management, internal medicine, acute care, or home-based experience
• Excellent communication and interpersonal skills
• Exceptional documentation and planning skills
• A collaborative and flexible spirit
• Strong computer skills (competence in Microsoft Office, particularly Word, basic Excel, Outlook, Teams, and familiarity with electronic medical records)

Pay Range

$79,560.00 - $109,200.00
