REGISTERED NURSE CARE MANAGER

Job Summary

The didgwalic Wellness Center is seeking a Dedicated Registered Nurse Care Manager to join their integrated care clinic that combines primary care, behavioral health, and opiate treatment. Duties/Responsibilities

• Partners with Providers, LPNs, and Medical Assistants in client/patient care.
• Aids in identifying patients with special health care needs/chronic diseases.
• Develops a method of tracking patient compliance with clinical guidelines and the plan of care.
• Engages clients in shared decision-making and collaborates to develop care plans with an empathetic and positive approach.
• Safeguards patient confidentiality as defined by HIPAA and the medical facility.
• Collaborates with didgwalic Leadership to establish objective measures of performance with a clear process for implementing CQI (Continuous Quality Improvement).
• Develops, conducts, and evaluates formal or informal in-services and programs focusing on the highest risk groups.
• Provides one-on-one educational support for each risk group or individual patient, aligned with best practice methodologies, Addiction Care Models, and trauma-informed care.
• Works with all departments to ensure clients' needs are met (e.g., housing, food insecurity, mental health).
• Maintains client files to meet all local, state, and federal program requirements.
• Integrates harm reduction principles and strategies to mitigate health risks and improve patient outcomes.

Qualifications

• Required Skills/Abilities: Knowledge of current medical practices in primary care, mental health, and addiction medicine; knowledge of information and techniques needed to diagnose and treat injuries, diseases, and deformities; empathy, personal and ethical boundaries; attention to detail; ability to balance competing priorities; cross-cultural understanding; understanding of trauma, historical, community, and personal factors in wellness and recovery; adherence to confidentiality regulations; effective communication; basic computer and software experience.
• Education and Experience: Must be a Licensed Registered Nurse in good standing with the State of Washington; five years’ experience of professional work as a Registered Nurse is desirable; Bachelor of Nursing (BSN) preferred; previous substance abuse treatment and/or primary care experience strongly preferred; Addiction Nursing Certification, or obtained within 1.5 years of employment in this role; BLS required.
• Physical Requirements: Prolonged periods sitting at a desk and working on a computer; prolonged periods of standing and bending; ability to lift 50 pounds at times; frequent exposure to chemicals, bloodborne pathogens, and patient body fluids.
