Registered Nurse Care Coordinator (RNCC) - Heart Clinic

As a Registered Nurse Care Coordinator in the Heart Clinic, you will play an integral role in helping patients successfully manage their care across the continuum. Working alongside nationally recognized cardiology specialists and an interdisciplinary team, you will coordinate care, provide patient education, support treatment planning, and help ensure exceptional outcomes for a diverse cardiovascular patient population. The Heart Center Clinic is a large, high-volume ambulatory cardiology practice that cares for approximately 170-200 patients each day and includes a multidisciplinary team of physicians, nurse practitioners, registered nurses, RN care coordinators, medical assistants, social workers, dietitians, and genetics professionals. Specialty services include General Cardiology, Electrophysiology, Advanced Heart Failure, Cardiac Transplant, Pulmonary Hypertension, Cardiovascular Genetics, Women's Heart Health, Sarcoidosis, and other highly specialized cardiovascular programs. This is a Monday-Friday Clinic with occasional early or later hours to accommodate patient needs.

In this role, you will serve as a key connector across patients, providers, and care teams, ensuring that patients receive timely, coordinated, evidence-based care throughout their healthcare journey. You will work in a collaborative environment where nurses are valued contributors and active partners in patient care.

Responsibilities

• Coordinating complex patient care activities across multiple providers, services, and treatment plans.
• Supporting medication management initiatives and clinical follow-up programs.
• Assisting with prior authorizations and facilitating access to necessary therapies and services.
• Conducting telephone-based clinical assessments and patient outreach.
• Managing patient registries and tracking care needs to support continuity and quality outcomes.
• Partnering closely with physicians, advanced practice providers, nurses, social workers, and other interdisciplinary colleagues to optimize patient care.
• Educating patients and families on disease management, treatment plans, medications, and available resources.
• Helping patients navigate complex cardiovascular care journeys while serving as a clinical advocate and trusted point of contact.

Requirements

• Education: Bachelor of Science in Nursing from an accredited nursing program.
• Experience: 1 year of registered nursing experience required, 3 years of related and relevant registered nursing experience strongly preferred.
• Specialty Board Certification strongly preferred.
• License/Certification: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification is required.

Physical Demands

• Job requires standing for prolonged periods, frequently traveling, and bending/stooping.
• Proficient communicative, auditory, and visual skills; Attention to detail and ability to write legibly.
• Ability to lift/push/pull 20 - 50 lbs.
• May be exposed to chemicals, blood/body fluids, and infectious diseases.

Pay

The pay range for this role is $85,820.80 - $113,401.60 annually. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
