About the Role

The primary role of a Registered Nurse (RN) Care Coordinator is to provide high-quality, holistic healthcare to patients with multiple chronic and preventive conditions (high-risk patients) through a patient-centered approach to improve their lives.

Responsibilities

  • Supports and empowers patients throughout the healthcare continuum, including holistic counseling, problem-solving “barriers to care” issues, and coaching clients on treatment and medication adherence, healthy life choices, and self-help behaviors.
  • Provides appropriate care and follow-up to high-risk patients.
  • Uses monthly reports to monitor patient outcomes, reviews results with providers, and takes appropriate follow-up action with patients.
  • Performs nursing assessment/medical triage of acute medical issues and medication side effects and refers or monitors appropriately.
  • Provides chronic disease and preventive screening education to clients, families, partners, internal staff, and external agencies in an organized and appropriate manner.
  • Assists clients in accessing support services by serving as a liaison with other community agencies and connects patients with financial assistance and entitlement resources as needed.
  • Manages Remote Patient Monitoring and Chronic Care Management.
  • Handles patient referrals and follow-up to other specialty medical, mental health, substance abuse, and community services.
  • Reads, analyzes, and interprets documents, reports, technical procedures, governmental regulations, and correspondence; writes routine reports and correspondence; presents information in one-on-one, small group, and large group situations.
  • Consults with physicians, healthcare providers, and outside agencies regarding continued care/treatment.
  • Uses the Electronic Medical Record system according to established guidelines.
  • Provides assistance with direct nursing interventions such as venipuncture, wound care, vaccinations, and other identified nursing responsibilities.
  • Ensures billable procedures are correctly documented and submitted in a timely manner.
  • Works with mathematical nursing concepts.
  • Performs other duties as assigned to support patient care, departmental operations, and organizational goals.

Requirements

  • Graduate from an accredited school of nursing and currently licensed as a Registered Nurse (RN) in the state of Iowa, able to work in the state of Iowa.
  • Two or more years’ experience as a registered nurse is preferred.
  • Excellent computer skills and ability to learn new systems.
  • Current in BLS (Basic Life Support) by the standards of the American Heart Association.
  • Proof of completion of Mandatory Reporter course at the time of hire or within 6 months of the start date.
  • Adherence to all training requirements as mandated by Federal, State, and Professional licensure and regulatory standards.
  • Understanding and compliance with Mahaska Health’s Compliance Program, including the Code of Conduct, Compliance Policy, Mahaska Health Experience Book, and all supporting policies.
  • Ability to perform the essential work functions of this job, with or without reasonable accommodation.
