Registered Nurse Care Coordinator

Responsibilities

• Supervise personal care workers
• Conduct home visits
• Assess consumers
• Develop and review care plans
• Obtain physician orders
• Evaluate consumer needs
• Maintain accurate records
• Ensure compliance with policies and regulations
• Document all care activities
• Provide supervision and ongoing training to personal care workers, including competency evaluations and performance reviews
• Coordinate services aligned with consumer preferences and advocate for consumer-controlled care
• Notify medical personnel of significant client changes and ensure timely documentation
• Assist with training new coordinators
• Participate in training sessions
• Support outreach activities
• Ensure infection control
• Manage schedules
• Support the agency's philosophy

Qualifications & Skills

• Current RN licensure
• Supervisory or administrative experience
• Strong communication skills
• Proficiency with computers
• Knowledge of Wisconsin Medicaid
• Experience in personal care, home health, or related fields (preferred)
• Flexibility
• Reliable transportation
• Ability to conduct home visits in varied environments (essential)
• Bilingual skills in English and another language (advantageous)
• Must meet background check requirements
• Adhere to applicable laws and regulations
