Registered Nurse - Care Coordination FT Days or Nights

Role Overview The RN Care Coordinator is responsible for assessing patients' transitional care needs, coordinating care across the continuum, and engaging with patients and families to ensure care goals are met. They manage discharge planning, facilitate interdisciplinary collaboration, and address barriers to timely discharge. The role involves conducting psychosocial and clinical assessments, developing individualized discharge plans, and ensuring effective communication with care teams, physicians, and community resources. The RN Care Coordinator also documents care activities, participates in interdisciplinary rounds, and supports professional development. Qualifications & Skills Candidates must hold an active RN license, BLS certification, and have at least one year of acute care nursing experience. Essential skills include excellent communication, assessment, organizational, and problem-solving abilities. Knowledge of regulations, community resources, and care coordination processes is required. The role demands self-confidence, maturity, and the ability to work effectively under stress in a fast-paced environment.
