Registered Nurse - Care Coordination FT Days or Nights

Role Overview

The RN Care Coordinator is responsible for assessing patients' transitional care needs, coordinating across the care continuum, and engaging with patients and families to ensure effective care planning and discharge.

Responsibilities

• Conduct psychosocial and functional assessments
• Develop individualized discharge plans
• Collaborate with physicians and care teams
• Facilitate communication regarding care progression
• Address barriers to timely discharge
• Manage post-acute referrals
• Participate in interdisciplinary rounds
• Ensure accurate documentation of all interactions and plans

Qualifications & Skills

• Associate or Bachelor of Nursing
• RN licensure (single or multi-state)
• BLS certification
• At least 1 year of acute care nursing experience
• Strong communication, assessment, organizational, and problem-solving abilities
• Knowledge of healthcare regulations and community resources
