Responsibilities

• Demonstrates competence in caring for patients with complex problems and complies with administrative policies and clinical protocols.
• Demonstrates clinical leadership skills through contribution to standards of care and division goals.
• Manages Cardiac Anticoagulation and Thrombosis Program clinical volume, education, patient, and family communications.
• Follows the Nursing Process to deliver safe care: assessment, planning, implementation, and evaluation.
• Completes home INR monitor applications, letters of medical necessity as needed.
• Finds and tracks outside labs for active anticoagulation patients.
• Completes Lovenox discharge best practices on all newly discharged patients.
• Communicates regularly with active anticoagulation patients for normal results and lab frequency.
• Provides reinforcement of Lovenox subQ teaching and administration for outpatients.
• Provides reinforcement of warfarin and DOAC teaching for outpatients.
• Provides clinical and administrative coordination to the program.
• Independently and without supervision provides oversight of administrative issues for clinics.
• Reviews patient schedule ahead of time and assembles all materials and records required by clinical staff for the clinic session.
• Identifies and brings pertinent issues to the attention of attending physicians based on results and records from a clinical perspective.
• Obtains missing records, including from sources outside CHOP.
• Follows up to obtain results of post-clinic studies for review by the clinical team.
• Schedules patients: assists in triaging new patients for urgency; ensures that patients are assigned appropriate time slots; assists in assigning new patients/consults to appropriate providers; and works with clerical staff to ensure coordination for patients in need of multiple clinic visits (Neuro-Oncology, Ophthalmology, MRI, etc.).
• Supervises scheduling process for patients.
• Coordinates transition of patients between the King of Prussia site and CHOP Main campus.
• First contact for new patient referrals (obtains medical history, arranges for receipt of medical records/scans/etc., and reviews for eligibility for clinical trials and other clinical research protocols).
• Maintains and enhances database.
• Maintains database by updating patient demographic and clinical information regularly, accurately, and independently.
• Runs a variety of queries and reports from the database.
• Maintains familiarity with the structure of the database and collaborates with Program personnel and Divisional computer specialists to enhance its scope and utility.
• Calls families, gathers information, and provides medical advice (when appropriate) when triage nurse refers a phone call.
• Contacts or refers calls to medical staff or advanced practice nurses when indicated.
• Follows-up with families by phone when clinically indicated (e.g. after discharge from hospital).
• Collaborates with medical staff and advanced practice nurses in patient care activities when possible.
• Coordinates studies and patient accrual.
• Tracks patients and patient results.
• Collects data as required by protocol.
• Aids in the development of case report forms.
• Enrolls appropriate patients on clinical studies.
• Provides information and instructions to study participants.
• Reviews informed consents with patients and families.
• Be an active member of the investigative team in program studies.
• Aids in the preparation of grant and foundation proposals.
• Assembles materials for submission to Stokes and ultimately to the funding source in a timely manner.
• Investigates and resolves simple administrative questions about grant requirements and structure.
• Aids in writing of new studies and protocols.
• Aids in designing study budgets.
• Assists in developing education materials for patients and families.
• Lectures to nursing staff regarding program topics.
• Lectures at regional patient/family symposia.

Qualifications

• Associate's Degree Nursing Required
• Bachelor's Degree Nursing Preferred
• At least one (1) year Experience in a clinical research setting.
• Excellent clinical, organizational, and communication skills.
• Knowledge of and ability to use standard computer programs.
• Knowledge of, or ability to learn, database programs, plus understanding of data management concepts.
• Basic understanding of academic, research, and/or clinical setting.
• Registered Nurse (Pennsylvania) - Pennsylvania State Licensing Board - upon hire
• Registered Nurse (New Jersey) - New Jersey State Licensing Board - upon hire

Pay

SALARY RANGE: $92,100.00 - $117,400.00 Annually
