About the role

The role is part-time and eligible for remote work. Candidates must have evening/night and weekend availability.

Responsibilities

  • Triage patient needs via telephone and during visits
  • Ensure timely access to appropriate services
  • Reduce care fragmentation
  • Promote optimal health outcomes and patient safety
  • Demonstrate professional clinical leadership and oversight
  • Efficiently triage patient needs
  • Conduct post-discharge calls for patients experiencing a transition of care
  • Document clinical findings
  • Utilize critical thinking skills to assess patient needs
  • Enter orders according to provider direction
  • Follow clinical and nursing protocols
  • Schedule patient appointments
  • Encourage patient portal usage
  • Provide nursing education
  • Consult with providers and other team members
  • Support physician and advanced practice providers with indirect work
  • Assess patients for gaps in care
  • Ensure patient medication list is accurate
  • Work with the care team to manage indirect patient work
  • Collaborate with insurance, DME, home care, and other care partners
  • Review patient medical history to support prior authorization
  • Assist patients with resources related to health needs
  • Participate in process improvement projects
  • Support clinical office/department staff
  • Perform clinical support tasks as outlined
  • Attend required meetings and training
  • Assume additional duties as required

Qualifications

  • Graduate of an accredited RN or LPN program
  • Valid, unrestricted RN or LPN license in the State of Michigan
  • Valid CPR certification
  • 1-2 years of clinical experience in a medical or physician office preferred

Skills

  • Competency in obtaining accurate patient vital statistics
  • Basic knowledge and understanding of medical terminology
  • Proficiency in operating a standard desktop and Windows-based computer system
  • Excellent communication skills in both written and verbal forms
  • Ability to work collaboratively in a team-oriented environment
  • Ability to work effectively with various levels of organizational members and diverse populations
  • Good organizational and time management skills
  • Ability to exercise sound judgement and problem-solving skills
  • Knowledge of the compliance aspects of clinical care and patient privacy
  • Ability to handle patient and organizational information in a confidential manner
  • Ability to travel to other office/practice sites and meeting and training locations
