Registered Nurse

Responsibilities

• Provide compassionate, high-quality clinical care to underserved patients
• Perform patient assessments and administer medications
• Support chronic disease management and care coordination across primary care, behavioral health, and wellness services
• Educate patients and document clinical interactions
• Collaborate with an interdisciplinary team to ensure holistic patient care
• Deliver patient-centered care, support interdisciplinary collaboration, and contribute to the organization’s mission of serving the community in a faith-based setting

Qualifications & Skills

• Registered Nurse licensure
• Strong clinical assessment and medication administration skills
• Experience or interest in chronic disease management and care coordination
• Excellent communication and teamwork abilities
• Commitment to health equity and serving underserved populations
