About the role

The Registered Nurse (RN) provides direct nursing care to agency clients under general direction, following agency policies and current standards of Nursing Practice. Responsibilities include assessing client health, developing care plans, and documenting all activities in the medical record.

Responsibilities

• Assess health status of assigned clients and develop a plan of care.
• Perform and document Nursing Intake/Medical History according to agency policy.
• Triage sick call lists, making referrals to appropriate medical practitioners.
• Complete follow-up assessments for clients treated for chronic or acute illnesses to monitor treatment effectiveness.
• Administer and assess PPD tests for TB screening.
• Monitor lab values, identify abnormalities, and report to attending physician or physician’s assistant.
• Review medical records to ensure continuity of care and assess care quality.
• Review medical records of clients being transferred to identify follow-up needs.

Requirements

• NYS RN licensure.
• BLS certification.
• Associate’s Degree in Nursing.
• Minimum of 1-3 years of experience as an RN, preferably with clients who have Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI).
• Available to work a flexible schedule as needed.
• Computer literacy, including proficiency in Microsoft Office Suite and EMR.
• Ability to work under pressure, multitask, and maintain excellent organizational skills.
• Experience working with diverse racial, ethnic, and socioeconomic backgrounds.
• Knowledge of medical terminology.
• Ability to work both independently and as part of a team.

Pay

$43.95-$49.02 per hour
