About the Role
Coordinate health care services for individuals in residential and day programs, support community-based settings for individuals with developmental disabilities, and work across multiple program sites in the Bronx, Westchester County, and Rockland County, NY.
Responsibilities
- Coordinate health care services for individuals in residential and day programs
- Issue medical referrals and follow up on health concerns
- Conduct staff training in medication administration, hygiene, infection control, and other health topics
- Administer medications and supervise AMAP-certified staff
- Participate in interdisciplinary team meetings and case reviews
- Maintain accurate medical documentation and MARs
- Provide emergency care and first aid
- Supervise LPNs and support less experienced RNs
- Transport individuals to medical appointments as needed
- Conduct assessments and provide in-services
- Support individuals with developmental disabilities in community-based settings
Requirements
- Graduate of an accredited professional nursing program
- Active RN license in New York State
- CPR/BLS Certification
- Valid NYS Driver's License (preferred)

- Strong oral and written communication
- Excellent organizational and interpersonal skills
- Ability to maintain confidentiality
- Familiarity with adaptive equipment and wheelchairs
- Flexibility to work across multiple program sites
Pay
$48–$50 per hour (based on experience)
Schedule
- Full-time: Monday–Friday, 9:00 AM–5:00 PM
- On-call availability during evenings or weekends as needed (remote support acceptable; does not require on-site presence)
