About the Role
Provide specialized nursing care for adult patients in an acute inpatient rehabilitation setting, focusing on recovery and transition planning.
Responsibilities
- Assess, plan, implement, and evaluate individualized care plans for patients.
- Administer medications, provide wound care, and support patient recovery.
- Educate patients and families about care procedures and recovery processes.
- Coordinate discharge planning and collaborate with an interdisciplinary rehab team to ensure comprehensive patient care.
Requirements
- Current RN licensure.
- Relevant clinical experience in inpatient rehabilitation.
- Strong assessment, critical thinking, and communication skills.
- Ability to work effectively within a team and deliver patient-centered care.
