About the Role

Provide primary and behavioral health nursing support in a community health setting focused on patient-centered care. Serve patients across all ages, including underserved populations, through assessments, triage, vaccinations, care coordination, and chronic disease management.

Responsibilities

• Conduct patient assessments and triage
• Administer vaccinations and manage chronic diseases
• Coordinate care and support integrated care planning
• Collaborate with providers, behavioral health, and dental staff
• Provide patient education and documentation in the EHR
• Support quality and safety initiatives
• Participate in care planning for underserved populations

Requirements

• Active nursing licensure
