About the role
We are seeking a compassionate Registered Nurse for Hospice Home Care to deliver patient-centered end-of-life care in patients' homes.
Responsibilities
- Performing comprehensive assessments
- Managing symptoms and pain
- Administering medications and treatments
- Developing individualized care plans to ensure comfort, dignity, and quality of life
- Educating and advocating for patients and families
- Collaborating with an interdisciplinary hospice team
- Documenting care in the EHR
Requirements
- Current licensure as a Registered Nurse
