Responsibilities
- Performing patient assessments and triage

- Supporting primary care providers and coordinating services across medical, dental, behavioral health, and wellness programs
- Providing patient education and documentation

Requirements
- Licensed Registered Nurse with current credentials

- Experience in a community or underserved setting is preferred
