This position is for a Registered Nurse who wants to continue using their clinical knowledge without providing direct or physical patient care. The RN in this role completes in-home visits by talking with clients and families, review health history, and develop clear, practical care plans for non-medical in-home caregivers to follow. The work is focused on assessment, communication, and care planning—not clinical tasks. What you will be doing Travel to clients’ homes to complete scheduled, non-clinical nursing assessmentsConduct verbal and observational assessments with clients and family membersReview medical history, diagnoses, medications, functional abilities, and daily routinesIdentify risks, support needs, and appropriate non-medical servicesDevelop and update individualized care plans for in-home caregivers to followServe as a clinical resource to the care team regarding the client’s plan of careMaintain accurate and timely documentation of assessments and care plans The focus of each visit is understanding the client’s health background, functional needs, and personal goals in order to build a realistic and supportive care plan.
