About the role
Provide direct participant care including acute triage, blood draws and wound care. Assist the care team for provision of participant care including after hours/weekend on-call rotation. Participate in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care and update changes as necessary. Attend Interdisciplinary Team (IDT) care plan meetings and Participant/Family meetings as applicable. May perform assessments in a variety of settings including PACE Clinic and/or Adult Day Center. Develop and provide education to participants, caregivers, and associates as necessary including end of life discussions. May require travel to participant's place of residence, long term care and assisted living facilities.

Requirements
- Licensure / Certification / Registration: Required Credential(s): BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
- Registered Nurse obtained prior to hire date or job transfer date.
- Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
Qualifications
- High School diploma or equivalent.
- Current BLS Provider certification.
- Current Registered Nurse license.
Skills
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of healthcare regulations and standards.
- Experience with patient care documentation and record-keeping.

$33.69 - $47.55 per hour
