Registered Nurse Assessment Coordinator - (RNAC)

Linwood Nursing and Rehabilitation Center, in Scranton, PA provides a full range of services in our highly regarded program of nursing and rehabilitation and nursing-centered care. We are currently seeking an experienced Registered Nurse Assessment Coordinator (RNAC) to join our collaborative team to support regulatory compliance, resident-centered care, and survey readiness. Responsibilities
- Conduct and coordinate the development and accurate completion of the resident assessment and care plan process in compliance with all federal, state, and local guidelines governing the facility.
- Demonstrate strong knowledge of MDS, the RAI, and the care planning process, including implementation of CAAs.
- Coordinate scheduling of OBRA and Medicare/managed care assessments, entry/re-entry records, and care plan reviews in accordance with the most recent regulatory guidelines.
- Inform residents, responsible parties, and staff of Medicare/managed care coverage status through appropriate documentation in accordance with all federal, state, and facility guidelines, providing updates as necessary.
- Work with Community Health Choice (CHC) case managers for Managed Medicaid providers.
- Participate in committees as directed.
- Apply in-depth knowledge of Pennsylvania case mix, the Medicare PDPM payment system, and requirements for managed care coordination.
- Utilize knowledge of Quality Measures.
Qualifications
- Associate or Bachelor's Degree in Nursing preferred.
- Previous experience in completion of Resident Minimum Data Sets and care planning preferred.
- Basic computer knowledge is essential.
- Strong organizational and communication skills.
Licensure/Certification
- Current RN license is required.
- Preferred certification: RAC-CT.
