Job Summary
Captures a patient-focused team in providing direct and collaborative health care in various clinical settings aimed at promoting and restoring optimal health across the lifespan. Utilizes the nursing process to provide care that respects individual, cultural, and religious diversity, and upholds patient rights. Manages case management to deliver comprehensive, coordinated, and seamless care. Facilitates the patient/provider relationship throughout the healthcare continuum.
Responsibilities
- Able to identify immediate and long-term patient care plans, involving patients and families in these plans.
- Establishes priorities for patients' needs, making decisions based on sound judgment.
- Maintains vigilance over patients' clinical status and identifies significant changes promptly.
- Communicates effectively with patients, family members, physicians, and the care team to ensure patient needs are met.
- Safely administers medications on schedule and according to hospital policies.
- Performs venipunctures and starts intravenous lines safely.
Requirements
- Active New York State Registered Nurse license.
- Bachelor's degree in Nursing preferred.
- Current BLS certification through the American Heart Association (AHA).
- At least 3 years of relevant experience, including ambulatory care.
