Responsibilities

  • Ensure all activities align with the vision of Elara Caring’s board of directors, executive team, and Hospice team leadership.
  • Assess hospice care patients and families to identify physical, psychosocial, and environmental needs, documented in clinical records and Interdisciplinary Team reports.
  • Obtain data on physical, psychological, social, and spiritual factors influencing patient/family/caregiver health status.
  • Communicate significant findings, problems, or changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, or relevant personnel.
  • Counsel, instruct, and include the patient, facility, and family in following the Interdisciplinary Plan of Care.
  • Teach hospice philosophy and pain/symptom management to staff in alternate care settings (e.g., nursing homes, hospitals, assisted-living facilities).
  • Provide appropriate support at time of death and perform bereavement assessments.

Requirements

  • Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
  • Current State License as a Registered Nurse (RN).
  • 1 year of experience in a clinical care setting.
  • Experience in a hospice or home health environment (preferred).
  • 50% travel required.
  • Ability to frequently lift, push, pull, and support up to 50 pounds (e.g., positioning or transferring patients, moving equipment).
  • Valid driver’s license, insurance, and reliable transportation to perform job tasks.

You will report to the Clinical Team Manager.
