About the role
Hospice care providers are needed to support patients and families facing end-of-life situations. Responsibilities include providing compassionate care, managing patient needs, and coordinating services.
Responsibilities
- Provide hospice care services to patients and their families
- Manage patient needs including medication administration, symptom management, and emotional support
- Care coordination with other healthcare professionals and family members
- Document patient care activities and communicate with interdisciplinary team
Requirements
- Bachelor's degree in a related field (nursing, social work, etc.)
- Current certification in hospice and palliative care
- Experience in healthcare or related field preferred
Qualifications
- Strong communication and interpersonal skills
- Ability to handle sensitive and confidential information
- Flexibility to work evenings, weekends, and holidays as needed
