RN Admissions (PRN)

About the role

Tidewell Hospice, a member of Empath Health, is seeking a compassionate, caring, and committed Admissions Registered Nurse (RN).

Responsibilities

• Assesses eligibility for Empath Health Network of Care while advocating for optimal access to the right care at the right time in any care setting.
• Completes the admission process based on care program specifics, guidelines, and patient-specific needs.
• Coordinates patient admissions in various settings including patient homes, long-term care facilities, hospitals, assisted-living facilities, and hospice houses.

Requirements

• Education and/or Experience: Associates degree (A.A.) or equivalent from two-year college or technical school; Minimum of or six months to one-year related experience and/or training; or equivalent combination of education and experience.
• Licensure: Valid Florida license as a Registered Nurse.
• Valid Florida State Driver’s License.

Qualifications

• Must provide proof of valid automobile insurance.
