Responsibilities
- Provides and directs nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
- Coordinates the total plan of care with other health care professionals involved in care.
- Ensures continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources.
- All duties performed within a Point of Care setting.
Requirements
- Graduate from a recognized, accredited school of nursing.
- RN - Registered Nurse - Georgia State Licensure and/or NLC/eNLC Multistate Licensure upon hire.
- CPR - Cardiac Pulmonary Resuscitation certification upon hire.
- Valid driver’s license, current vehicle insurance, and access to a dependable vehicle or public transportation.
