Registered Nurse 12HR, Full time Nights, Critical Care Unit

About the role

Responsible for coordination of care to meet identified needs of the patient and family. Supervises the management of patient care utilizing critical thinking and the nursing process, discharge planning, resource identification, linking and scheduling patients with required services, monitoring care delivery, advocating for the patient, care planning, and evaluating outcomes. Coordinates patient care across the health care continuum and assists other team members with meeting individual patient needs.

Responsibilities

• Care of the patient receiving Sedation/Analgesia
• Care of the patient with Hemodynamic monitoring and/or support
• Care of the patient with Neurological monitoring
• Care of the patient with Pulmonary monitoring and/or support, including ventilators
• Care of the Trauma patient
• Demonstrates competent knowledge base in pathophysiology of the cardiovascular patient through appropriate assessment, critical thinking skills, and plan of care, addressing:
  • Organ perfusion and cardiac output
  • Patient activity
  • Dysrhythmias
• Demonstrates competent knowledge base in the pathophysiology of the neuro patient through appropriate assessment and critical thinking skills, formulating and implementing an ongoing patient-specific plan of care addressing:
  • Adequate cerebral perfusion pressures
  • Patient safety
  • Appropriate levels of stimulation for the patient
• Demonstrates competent knowledge base in the pathophysiology of the respiratory patient through appropriate assessment and critical thinking skills, formulating and implementing an ongoing patient-specific plan of care addressing:
  • Maintenance of normal or appropriate O₂/CO₂ levels
  • Identification and intervention in emergent situations
  • Weaning assessment with appropriate sedation and daily Rapid Shallow Breathing Index (RSBI) for mechanically ventilated patients
• Demonstrates competent knowledge base in the pathophysiology of the metabolic patient through appropriate assessment and critical thinking skills, formulating and implementing an ongoing patient-specific plan of care addressing:
  • Monitoring and interpreting acid-base balance and lab values
  • Patient’s nutritional status
  • Optimal glucose control (80–110)
• Demonstrates competent knowledge base for all current and future unit and hospital-specific core measures to improve patient outcomes through appropriate assessment and critical thinking skills, adhering to:
  • Ventilator Bundle on all mechanically ventilated patients to reduce ventilator-acquired pneumonia risk
  • Bloodstream infection prevention standards to reduce central line catheter BSI, including:
    • Use of central line cart
    • Adherence to hand hygiene
    • Full barrier precautions
  • Sepsis bundle using “Trigger Tool” to identify suspected/identified critically ill patients for early goal-directed therapy
  • Appropriate sedation management and use of approved Sedation-Agitation Scale
  • Optimal pain management set to the patient’s functional pain goal per policy
  • Patient-centered care philosophy, including access for patients’ loved ones
  • Palliative care bundle, supportive care, and communication
• Demonstrates technical skills to safely operate and troubleshoot critical care equipment and diagnostic devices
• Demonstrates proficiency in delivering age-specific care to assigned patient populations: adolescent, adult, and geriatric in the critical care setting
• Ensures environment safety by:
  • Maintaining clear hallways and closed fire doors
  • Locking medication room/supply doors
  • Discarding outdated nourishment
  • Documenting refrigerator temperatures on designated logs
  • Reporting and following up on needed equipment repairs
• Provides clerical support by:
  • Demonstrating proficiency in use of information systems (Meditech, PCS, Acuity, KRONOS, and email)
  • Making accurate entries in each system
  • Performing chart assembly and breakdown
  • Ensuring accurate and complete documentation in PCS and CCU Flow-sheet
  • Maintaining medical records with patient identification
• Attends Annual Skills Day and all mandatory classes per hospital policy or unit needs
• Is knowledgeable about patient safety, core measures, TJC requirements, and Patient Safety Goals as they pertain to the role
• Approves timecard on time, maintains timecard, and completes all required logs (missed meal/break)
• Actively participates in unit cleanliness

Requirements

• Bachelor’s degree in Nursing (BSN) required. Associate Degree in Nursing (ADN) considered if enrolled in a BSN program with degree completion within 2 years of hire or transfer to an RN position
• Diploma or transcripts on highest degree earned required
• Minimum 2 years of current experience in a critical care unit; acute mixed unit preferred
• 3 years of experience preferred
• Surgical, Neuro, Cardiac, and Trauma nursing experience strongly preferred
• Must challenge and successfully pass the Basic Dysrhythmia course curriculum within 21 days of hire
• Basic computer skills; experience with computerized documentation systems required
• EPIC experience preferred

Licenses & Certifications

• Current unrestricted Registered Nurse (RN) license issued by California Board of Registered Nursing (BRN)
• Current Basic Life Support Provider (BLS) issued by American Heart Association
• Current Advanced Cardiovascular Life Support Provider (ACLS) issued by American Heart Association
• Current National Institutes of Health Stroke Scale (NIHSS) certification within 30 days of hire/transfer

Skills

Pay

The estimated base rate for this position is $50.30 – $77.95 per hour. Additional individual compensation may be available through differentials, extra shift incentives, and bonuses. Base pay is only a portion of the total rewards package.

Compensation structure includes the following premiums for every shift:

• Base rate paid for the first 8 hours of work in a day
• 1.5× overtime pay for hours 8–12 worked in a workday
• 2× double-time pay for any hours beyond 12 in a workday

The estimated blended hourly rate, accounting for these premiums on a 12-hour shift, ranges from $58.68 to $90.94.
