Duties & Responsibilities
- Assesses nutritional status, identifies nutrition-related needs, and develops and implements individualized nutrition care plans and interventions for TrueCare patients or WIC program participants in accordance with the Nutrition Care Process, as established by the Academy of Nutrition and Dietetics and in compliance with TrueCare or CDPH/WIC policies and procedures.
- Conducts group or individual nutrition education and support using Patient-Centered Education (PCE) principles to support optimal health outcomes.
- Delivers in-service and outreach nutrition education for TrueCare staff, interdisciplinary care teams, and physicians ensuring alignment with clinical standards and evidence-based practice.
- Refers TrueCare patients or WIC program participants to other TrueCare services and community programs; documents referrals and follow-up in accordance with policies and procedures.
- Mentors TrueCare and WIC staff, dietitians, dietetic interns, and volunteers.
- Attends, completes, and actively participates in department meetings, team huddles, continuing education, and trainings, as assigned.
- Flexibility to work at assigned TrueCare or WIC locations based on department needs and/or goals.
- Ensures punctual, regular, and timely attendance in accordance with the TrueCare Employee Handbook; report to work on time and as scheduled and remain at work for the entire duration of each shift; and follow TrueCare policies for notifying the supervisor of and documenting all planned and unplanned absences.
Qualifications
- Master’s degree from an accredited institution in nutrition, dietetics, or a related field. (Dietitians licensed prior to January 1, 2024 must hold a minimum of a bachelor’s degree in nutrition, dietetics, or a related field.)
- Current registration with the Commission on Dietetic Registration (CDR).
- Basic knowledge of conversational Spanish and willing to learn to counsel in Spanish.
- Strong typing and computer skills with proficiency in Microsoft Office applications (Outlook, Word, Excel, PowerPoint, and Teams).
Desired Qualifications
- Knowledge of the WIC program or CPSP program guidelines.
- Bilingual in English and Spanish.
